Hospice is a philosophy of care with an emphasis on improving quality of life, Hospice provides a framework for continuous, multi disciplinary care. Our hospice services focus on quality of life, emotional support for the entire family, in-home management of medical needs, and the advanced management of pain and symptoms.

Yes, our staff are on-call and available 24/7 during emergencies. 

The sooner we start, the more effectively we can help enhance the quality of life for our patients and their families. Our staff is available to work with the family and physician to determine when a patient can benefit the most from our hospice care.

Hospice care is for patients who are terminally ill or have a life-threatening illness determined by a physician. These patients include, but are not limited to those having diagnoses such as Cancer, end stage lung disease, liver disease, kidney disease ect. 

Hospice services are designed to provide support to patients, their families and loved ones wherever they  call home. This can be a Long Term Care Facility, Assisted Living, Retirement Community or at the patients own home. The goal is to maximize the quality of the patients life . 

The hospice is responsible for all equipment and supplies and medications that pertain to the patient’s primary diagnosis and manage pain.

Yes, the hospice ensures patients have the supplies and equipment they need for comfortable living (e.g., wheelchair, hospital bed, incontinence supplies, hygiene products, etc.)

If you have Medicare Part A, you can get hospice care benefits if:

• You receive care from a Medicare-certified hospice
• Your attending physician (if you have one) and the hospice physician certifies you as terminally ill, with a medical prognosis of 6 months or less to live if the illness runs its normal course
• You sign an election statement to elect the hospice benefit and waive all rights to Medicare payments for the terminal illness and related conditions

Medicaid provides hospice coverage, but it varies by state. Medicaid beneficiaries should check with their state Medicaid agency and state hospice association for specifics about what services are covered.

Medicare coverage includes: *

• All items and services needed for pain relief and symptom management
• Medical, nursing, and social services
• Drugs for pain management
• Durable medical equipment for pain relief and symptom management
• Aide and homemaker services
• Other covered services you need to manage your pain and other symptoms, as well as spiritual and grief counseling for you and your family

Patients with a private or employer-provided health plan should check with their insurance provider for details about hospice eligibility, coverage and out-of-pocket expenses.
